We are a local community of healthcare providers who seek to 'normalize" the benefits of utilizing end-of-life hospice care. We believe that cultural nuances, community ties, values, and traditions are essential aspects of how individuals transition back to spirit.
We work in excellence and in love to serve the families who have chosen to have a peaceful departure to eternal rest. With support to patients and caregivers, we work to ensure clinical and psychosocial needs meeting the exact desires and expectations of each family.
For each family, this is a unique experience. However, we are committed to respecting the process families CHOOSE and to support everyone on the journey. We serve patients in their homes, nursing homes, assisted living facilities, or wherever you are with 24/7 access to care as needed.
We exist as a COMMUNITY in the commonality that as we all live, we all must also die. Our team has chosen to be guardians for the dignity of that transition. More than 90% of people surveyed say they want to die at home. We customize care plans for each family based needs to support clinical, social and spirituals requirements.

Another way to inquire about hospice is to talk with your physician, and he or she can make a referral to hospice. Death is inevitable but discussing it and how to face it is challenging because, in most communities and families, it’s a taboo topic. The Lighthouse Community Hospice shines a light on the process, expectations, and end-of-life choices so that the entire family feels supported during this emotional time. Our team consists of physicians, nurse practitioners, nurses, certified nursing assistants, social workers, religious/spiritual counselors, and volunteers.

Our primary goal is to ensure you or your loved one is as comfortable as possible during this time and that you transition the way that YOU choose while in hospice care. Therefore, we assess, evaluate, and update our care plans regularly to ensure success in meeting this goal. We consider the scope of your physical, psychosocial, social, and spiritual well-being to include respite care. We also address family and caregiver needs. We ensure that you can be supported and that your directives are executed as you have desired consistent with clinical, cultural, and ethical standards.
We Care. We listen to your concerns, and then we educate you on your choices regarding them. We are present in your time of need. That’s how we differentiate ourselves from others providing hospice and palliative care services. Consider us when you have a life-limiting illness, and you want ownership of the ‘process’ and the choices of how you will transition.
